Qualifications for Services
To receive medical services, you must bring:1. Proof that you live in Irving (driver’s license, lease agreement, or utility bill)
2. Proof that family income does not exceed 150% of the federal poverty line (check stubs, income tax return, letter from employer)

Family of 3 - up to $26,000/yr
Family of 2 - up to $20,000/yr
Individual - up to $14,000/yr
